registration You will need to come into the Health Centre to register, as the forms have to be signed (a parent or legal guardian can sign for a child under 16 years of age). If you have your Medical Card from your previous doctor, please bring this with you.

You will be registered with the doctor who has the least patients during the month in which you apply to join the practice. This is for administrative purposes only and you may see whichever doctor you wish. When phoning for an appointment please specify if you have a preference as to which doctor you see. We will endeavour to meet your request although you may have to wait a little longer for an appointment with a specified doctor.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
